Job Title/Position:       Family Support Volunteer

Reports to:                     Melissa Justice, Volunteer Coordinator

 JOB DESCRIPTION SUMMARY

The Family Support Volunteer provides companionship and support services according to his/her experience and training while in compliance with organizational policies and the patients plan of care.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  1. Serves as an integral member of the interdisciplinary group amid various settings: home care; respite; inpatient; bereavement and/or administrative office duties.
  2. Participates in general orientation.
  3. Serves as a substitute family/caregiver in the home performing activities that the volunteer has been prepared for and has agreed to perform. The volunteer may do homemaker chores, run errands, and provide respite care.
  4. Serves the patient and family/caregiver under the guidance of Volunteer Coordinator and Case Manager or as part of the administrative team.
  5. Documents as educated and instructed while maintaining confidentiality.
  6. Meets all health requirements set by Hospice of Amador and Calaveras.
  7. Provides availability on a regular basis and/or keeping informed with the patient and family/caregiver needs.
  8. Participates in monthly volunteer meetings and educational in-services.
  9. Maintains open communication and reports regularly to the Volunteer Coordinator.
  10. Assignment of other duties (as needed) directed by the Volunteer Coordinator.
  11. Participates in quality assessment performance improvement teams and activities.
  12. Participates in interdisciplinary groups meetings, as appropriate.
  13. Participates in yearly performance evaluations and improves certain areas as needed.

POSITION QUALIFICATIONS

  • Ability to work within the interdisciplinary group
  • Clear criminal background checks
  • Ability to provide proof of a current California Driver’s License and proof of auto insurance

Hospice of Amador & Calaveras Job Description

POSITION TITLE: Registered Nurse Case Manager

REPORTS TO: Director of Patient Care Services

POSITION SUMMARY
Responsible as a member of Interdisciplinary Team (IDT) to the Director of Patient Care Services (D
of PCS) and to the attending physician. The RN Case Manager provides skilled nursing assessment,
planning and interventions to maximize the comfort, health, safety and quality of life of patients and
families, consistent with Hospice of Amador policies, the IDT Plan of Care (POC) and in compliance
with all state and federal regulations. He/she is responsible for the completion of assigned home
visits, and admission or informational visits, based on patient/family needs. Confers with the
Director of Patient Care Services and the IDT members regarding cost effective interventions and
oversees the management of covered services to stay within budgetary guidelines. This position
requires a minimum of four On-Call shifts per month, including evenings, nights, weekends and
holidays. This is a full time, non-exempt staff level benefited position.
This position will be evaluated on the adequacy, appropriateness and timeliness of the following
criteria:
AREAS OF RESPONSIBILITY/ACCOUNTABILITY CLINICAL SERVICES
1. Assumes primary responsibility for a patient/family caseload and responds to
patient/family needs in accordance with the POC.
2. Assesses and admits patients, develops a comprehensive POC with input from IDT
and completes required documentation in accordance with Hospice of Amador
policies and procedures.
3. Obtains data on physical, psychological, social and spiritual factors that may
influence patient/family health status and incorporates that data into the POC.
4. Responds to medical or emotional crises and/or death of patient and makes home
visits to provide direct care as appropriate. In On-Call situations, responds to caller
within 30 minutes of contact to answering service.
5. Maintains current knowledge and practice of universal precautions and maintains
infection control supplies in car stock at all times.
6. When On-Call, utilizes knowledge of nursing practice, home health care, crisis
intervention and family dynamics to triage phone calls and determine need for a
home visit.
7. Works effectively as a patient advocate assuring patient rights.
8. Assists in providing 24-hour, seven days a week On-Call coverage to
patients/families.
9. Accepts other assignments as requested by the Director of Patient Care Services or
her designee.
10. Demonstrates knowledge of medications, including indications, dosage ranges, side
effects and potential toxicity and administers all medications in a safe manner.
11. Reports any medication errors promptly.
12. Provides home visits at time of death to pronounce death, and to notify the physician,
other IDT members and contracted vendors.
13. Disposes of all controlled substances after a patient’s death, according to agency
policy.
14. Demonstrates ability to prioritize and complete tasks with time frames mandated by
agency policy and state and federal regulations.
15. Assists other Case Managers or staff nurses by assuming responsibility for their
assigned patients when requested by the D of PCS.
16. Completes a nursing physical assessment within two weeks of the beginning of every
certification period or delegates the assessment to another staff nurse.
17. Updates On-Call Books as to last M.D. visit in order to determine if patient is a
Coroner’s Case.
18. Updates care plans at least twice monthly after IDT meetings and as needed.
19. Assists the billing person and/or Director of Patient Care Services to negotiate rates
and type of reimbursement for patients who have private insurance. Informs IDT of
covered and non-covered services. Notifies coroner’s office when a patient dies with
an infectious disease.
20. Completes or delegates Home Health Aide supervision at least every two weeks.
21. Completes bereavement assessment and/or follow-up as requested by the
Bereavement Coordinator or the D of PCS.
22. Ensures patient’s and family’s right to confidentiality in all verbal and written
communication.
23. Ensures coordination and continuity of care between inpatient and home care
settings.
INTERDISCIPLINARY TEAM COLLABORATION AND COMMUNICATION
1. Initiates communication with attending physicians and community resources to report
a change in patient status and to coordinate optimal care for the patient/family.
2. Participates in IDT meetings to ensure continuity of patient care services.
3. Collaborates with IDT members for discussion, consultation and direction and makes
referrals to appropriate IDT members to follow up on patient concerns and issues.
4. Demonstrates ability to communicate effectively both verbally and in writing
5. Makes referrals to appropriate IDT members to follow up on patient concerns/issues.
6. Documents all care provided in an acceptable format and in accordance with agency
timeframes.
7. Maintains regular communication with the Director of Patient Care Services.
8. Maintains regular communication with the attending physician regarding POC.
9. Maintains up to date patient medical records so that problems, plans, interventions
and goals are accurately and clearly stated and changes are reflected as they occur.
10. Meets regularly with other IDT members to review problems or unique issues from
caseloads, share professional support, and exchange feedback .
11. Attends monthly nurse’s meetings.
12. Responsible for all information contained in communication book, acknowledging
that it has been read by initialing each page.
13. Provides report of all On-Call activities to the next nurse scheduled for On-Call on
weekends and holidays by 8:00 AM or to IDT member in office on weekday
mornings by 9:00 AM.
14. When on-call, notifies Administrative On-Call person with any changes in Medicare
Benefit level of care, i.e. continuous, respite, or inpatient hospitalization.
15. Informs the Director of Patient Care Services of unusual or potentially problematic
patient/family issues.
16. Completes Discharge Summaries and forwards to physicians within 20 working days
of discharge.
17. Acts as a liaison from Hospice of Amador to contracted facilities and vendors.
18. Conforms to scheduled work hours with punctuality and regular attendance, notifying
supervisor in advance of planned absences.
19. Works cooperatively with all IDT members.
EDUCATION OF PATIENTS, FAMILIES, CAREGIVERS AND STAFF MEMBERS
1. Provides phone triage, instruction and support to patients/families in accordance with
accepted Standards of Practice.
2. Instructs primary caregivers, volunteers and employed caregivers to provide care as
indicated.
3. Educates patients/families, physicians, referral sources and the community about
hospice philosophy and Hospice of Amador’s services.
4. Participates in the orientation of all new employees and volunteers as requested.
5. Participates in community programs as requested.
QUALITY ASSURANCE /QUALITY IMPROVEMENT
1. Demonstrates knowledge of the patient related policies of the agency and state and
federal regulations, which relate to the delivery of hospice services.
2. Participates in Quality Assurance/Quality Improvement (QA/QI) activities as
requested.
3. Completes all medical record documentation in a timely and legible manner.
PROFESSIONAL AND PERSONAL GROWTH
1. Attends mandatory inservices.
2. Completes self-evaluation annually to identify areas where improvement is needed
and develops a plan to accomplish improvement.
3. Develops annual goals and implements plan to accomplish goals.
4. Responsible for maintaining California licensure requirements.
JOB QUALIFICATIONS
1. Registered Nurse with current California nursing license.
2. Previous experience in hospice, home care, oncology or acute care setting required.
3. Must have a valid California driver’s license and an automobile that is insured in
accordance with state and agency requirements and is in good working order.
4. Current CPR certification.
5. Available to work week-ends, nights and holidays.
6. Ability to organize and prioritize tasks and to work independently.
PHYSICAL AND OTHER QUALIFICATIONS
1. Ability to safely lift up to 10 pounds regularly without assistance and to frequently
assist caregivers or other staff with transferring and ambulating patients weighing
100 pounds or more.
2. Ability to use hands to handle, palpate and reach
3. Ability to stand, walk, stoop, kneel, crouch, turn, or twist periodically throughout the
workday.
4. Ability to climb stairs and walk up hill occasionally to access patients’ homes.
5. Ability to sit for up to two-thirds of the work day.
6. Ability to safely operate a motor vehicle frequently throughout the day.
7. Sensory ability to hear, speak and to effectively communicate with staff, patients and
family members
8. Cognitive ability to read, interpret and effectively utilize information from
instructions, policy and procedure manuals and government regulations, and to
follow verbal instructions.
9. Ability to tolerate occasional exposure to outside weather conditions.
10. Ability to tolerate occasional exposure to second hand smoke in patient’s home.
